Marriage, Birth and Death Certificates are $20.00 for a certified copy. Only certified copies will be issued.

To obtain Certified copies of Death, Marriage and Civil Union certificates please send a request including the names of the parties, date of the event and $20.00 for each certified copy along with a self addressed stamped envelope to:

To obtain a certified copy your Birth Certificate please fill out the Application for Birth Certificate and enclose a photo copy of your driver's license. Mail it to the above address along with $20.00 and a self addressed stamped envelope.
